INSURANCE CLAIM FORMS
These are the forms to be used to file a claim with Health Special Risk (HSR) when an injury occurs during an approved Rodeo or Activity. You will need the signature of the Rodeo Secretary or the National Director on the bottom of the front page to verify that the member was a participant of the rodeo or activity.   Please read all pages of this form and file all appropriate paperwork timely.
If you have filed a claim and are having any problems, there is a number on the first page as well as a Customer Service number on the 3rd page to call with questions.
